Request for Proposal for Lawn Services
 
INTRODUCTION
 
Gilmer County Health Department (GCHD) is soliciting proposals for a vendor for lawn maintenance services for their location at:
 
28 Southside Church Street
Ellijay, GA 30540
 
GENERAL CONDITIONS
 
  1. Hours:
Lawn care maintenance shall be performed after health department hours of operation unless prior arrangements have been made with management. The health department hours of operation are as follows:
 
Monday, Wednesday, Thursday: 7:30 AM - 4:30 PM
Tuesday: 7:30 AM - 7:00 PM
Friday: 7:30 AM - 1:00 PM
 
  1. Terms of Contract:
The initial contract term will be for one year from the date of award. The contract will be renewable on an annual basis for up to two (2) additional years. All subsequent renewal agreements shall incorporate all terms, conditions, and specifications contained in original agreement, unless mutually amended in writing.
 
  1. Insurance Coverage:
During the term of the contract, the Contractor at their sole cost and expense shall provide commercial insurance of such type and with such terms and limits as may be reasonable associated with the contract. This includes Workers’ Compensation, General Liability and Automobile.
 
SCOPE OF SERVICES TO BE PROVIDED
 
Routine lawn maintenance service will be provided every 2 weeks from April through October for a total of 14 visits per year. Any visits required more than the 14 per year may be billed at a pre-determined fee which must be included in the proposal. Vendor must provide the following services:
    1. Mow and trim all grass areas
    2. Pick up paper and debris from parking lot
    3. Blow all grass trimmings from parking lot/sidewalk.
    4. Edge all curbs/sidewalks.
    5. Put down fresh much and pull all dead flowers/weeds twice a year (April and October)
    6. Trim all shrubs around building one time per year.

RESCHEDULED CLINIC

Rescheduled Woodstock Drive Thru Flu Shot Clinic webWoodstock, GA – The public health DRIVE-THRU FLU SHOT CLINIC that was scheduled earlier in Woodstock but postponed due to weather is now rescheduled for Tuesday, October 19th from 9 AM to 2 PM. The drive-thru clinic location will now be at the Cherokee County Health Department Woodstock Health Center at 7545 North Main Street, Woodstock, GA 30188. The shot is no-cost if covered by one of several health plans. If paying out of pocket, the cost is still relatively low at $25 for regular flu vaccine and $65 for high dose flu vaccine for people aged 65 and older. Both vaccines guard against 4 different strains of flu. Cash, checks, credit cards, Medicare, Medicaid, Aetna, Anthem Blue Cross Blue Shield, Cigna, Coventry, Humana, Ambetter and United Healthcare Insurance will be accepted.
