Case Management

Each Living Bridge Center patient has their own medical case manager. They work with you as part of a team to identify and address any and all barriers to quality medical care.

Together, you and your case manager will review your treatment plans, progress made toward healthier living and what other services can be provided to make adherence easier.

Your case manager can help you access other needed services, will follow up with you on all related referral medical appointments. Need assistance with getting to your medical appointments? Your case manager can arrange for travel to and from the appointment. Need assistance access food resources? Your case manager can help.
